Appointment Booking Form for a Clinic Website: What to Ask

GrowwChat Team ·

Most clinic websites ask patients to phone or fill in a form with a dozen fields. Many people leave at that point, especially on a phone, and the clinic never knows they were interested. A short booking flow fixes most of this.

Ask only what the front desk needs to confirm a visit: the reason for the visit as a choice (checkup, pain, follow-up, other), the patient name, a mobile number, and a preferred day and time. Everything else, such as medical history or insurance details, can wait for the visit or a follow-up call.

Start with the reason for the visit, because it is the easiest question and it lets you route the request. A dental clinic can send a pain request to the first free slot and a cleaning request to a regular slot. Branching like this is where a chat form beats a long page of fields.

Put the date and time picker inside the conversation instead of sending people to a separate calendar page. Show only the slots you can actually serve, and confirm the choice back to the patient in plain words.

Close with what happens next: who will call, from which number, and by when. Add a reminder that the form is not for emergencies. Then send the response to the person at the desk by email, and keep a spreadsheet or CRM connector for the record.

Privacy matters more for clinics than for most sites. Collect the minimum, say why you ask, and do not ask for diagnoses in a form unless you are allowed to store them. Check the rules that apply in your country before you collect health data.

To try this, open the Multispeciality Clinic template in the template gallery, change the specialties to your own, and publish it as a link or widget. Test it on your own phone before you share it.
